3. Online appointment requests
Submitting an online booking form is a request and does not by itself guarantee or confirm an appointment. An appointment is confirmed only when accepted by the Clinic. Schedules may change because of clinical needs, emergencies, availability, holidays, or circumstances beyond our reasonable control. You are responsible for providing accurate contact and appointment information and for informing the Clinic if you need to cancel or reschedule.
4. Patient portal and account security
You may use the patient portal only to access information that belongs to you or that you are legally authorized to access. You are responsible for keeping login details confidential, using a secure device, signing out after use, and notifying the Clinic promptly if you suspect unauthorized access. You must not share another person’s records, impersonate another user, or attempt to bypass any security measure.
